Preceptor Orientation Manual (.pdf) PRECEPTOR FORMS The following forms are provided for identifying and approving preceptors:
Students planning to take a clinical or practicum course Spring 2009 must submit completed preceptor forms by November 24, 2008. Preceptor forms should be forwarded to Lori Stephens by fax at (615) 366-3953, by email at lori.stephens@tbr.edu or by mail to 1415 Murfreesboro Pike, Suite 350, Nashville, TN 37217. Students are required to keep a copy of all forms for their records and may be requested to upload forms into their course at the beginning of each semester. For questions and assistance regarding clinical/practicum placement and form deadlines, please contact Lori Stephens at lori.stephens@tbr.edu. |
